Addiction Case Management and Recovery Coordination

Treatment admission does not automatically create continuity. The Firm organizes communication, accountability, transitions, and family decisions across the recovery system.

One accountable map across moving parts

Information becomes fragmented when treatment, medical care, psychiatry, family, legal obligations, work, school, housing, monitoring, and coaching operate separately. Case management establishes goals, roles, communication protocols, and a practical sequence.

What scope may include

The Firm coordinates authorized professionals without replacing their licensed roles.

  • Treatment and provider communication
  • Transfer, discharge, and step-down planning
  • Return to work, school, home, or independent living
  • Monitoring, coaching, and sober-support coordination
  • Family communication and boundary follow-through
  • Review of recommendations, barriers, and emerging risk

Independent guidance

Scope and fees are set after assessment because a weekly coordination engagement differs from an urgent, travel-intensive case. Case management is not psychotherapy, medical care, legal representation, or emergency response.

Common questions

Who receives updates?

The communication plan is defined at the start and follows applicable consent, privacy, contractual, and professional requirements.

How long does case management last?

Duration depends on complexity, transitions, stability, and the agreed scope. It should be reviewed against clear goals rather than continuing without purpose.
